Helping a member advocate for her own well-being

Getting discharged doesn't always leave patients with a sense of relief. Instead, it can present a new set of challenges, such as adapting to new routines and medications. Without the right support, many people find themselves back in the hospital due to infection, stress-related incidents, or adverse drug events. To give people the help they need, Accolade’s intelligent technology empowers our team to proactively support members after they leave the hospital.

When a member was discharged after her hospitalization for a gastrointestinal bleed, our internal system went to work. It notified one of our nurses, who reached out to the member about her hospitalization, discharge, and ongoing symptoms. We listened to her concerns, helped her get in to see her primary care provider, and offered post-visit guidance. When her symptoms persisted, we helped her schedule a re-evaluation, connect with a pulmonologist, and find answers to her questions. We also worked with her to obtain a portable oxygen device and request work accommodations, so she could get back to working and living.

“Having a conversation with us and helping us help you with your provider is the best way to get the care that you know you deserve,” said Accolade solutions consultant, LaNara Scruggs.
